A fire report form can be obtained by calling the
Fire Prevention Bureau at 614.645.7641 or downloaded FPB Form. Please have date of the incident or
the incident number available. The fire report will be sent to you within
three (3) business days of your request. Please be advise it usually takes
five (5) business days for the report to be received at the Fire Prevention
Bureau Office.
