How do I get an EMS Report? |

EMS Records are only available to the patient. They are considered to be
private and confidential medical reports. A patient may request a report in
person by signing a
medical authorization
and showing ID. They are also available by subpoena. Generally
attorneys request these reports with medical authorization from their clients. Please contact 645-7384 for details.
